Suggest Treatment For Persistent Bacterial Infection

Patient is having difficult to recover from infectious from Phenomena he is diabetic was on three kind of antibiotic Zosyn, Vancomicin, & Bactrium because of that reason he can not get wean down from Ventilator that is main issue ? what can you suggest there are not LTAC hospital in bay area what can be expected ? Please advice

As he has s a diabetic and on ventilator he needs continuous monitoring of blood sugar along with continuing his treatment for the infection with the antibiotics.
I suggest to get blood culture and sensitivity for knowing the correct antibiotics for him.
