Suggest Treatment For Pulmonary Tuberculosis

hi I am a 57 yr old female.my xray says there is subtle fibronodular change within the right pulmonary apex and generalized increase of the broncovascular markings with thickening of the bronchial walls.the former represents scarring due to previous tuberculous infection .the latter could reflect bronchitis but if there are clinical signs of pulmonary tuberculosous biochemical and bacteriological correlation regarding endobronchial spread of PTB is suggested as this entity can present with above described radiographic manifestations .

Thanls for your question on HCM.
In my opinion your x ray finding are suggestive of old healed lesion. This can be due to
1. Old TB
2. Old pneumonia.
So bacteriological evidence of TB should be there before starting TB Treatment.
So I advice you following investigations.
1. CT thorax with contrast as CT gives much better idea than chest x ray about lung parenchyma.
2. Sputum examination for AFB.
3. If sputum is negative than Bronchoscopy and BAL analysis for TB.
If all the above are normal than no need to worry about TB at present.
