Suggest Treatment For Rapid Breathing And Cough

Thank you Doctor for your services providing on humanitarian grounds , I, am belonging to Pakistan’s Sindh Province and District Dadu my email address is YYYY@YYYY cell No.0000 I, am an Officer at National Bank of Pakistan I suffered from TB in the year 2008 I took medicines (3 tables myran) for six months but not recovered, and again get treatment from other Doctors but up to 12 months could not recovered and became very week during the time not suffered from heomoplysis but suffered from high respiration and cough. Admitted at Liaqat National Hospital Karachi, a university teaching hospital in February 2009 thereat Doctor informed as MDR Pulmonary koch’s and on 2nd line . After a treatment of six months the AFB semeir as well as culture reports showed the negative position . but the Doctors kept me on ATT of 24 months (due to my age of 57 years at that time) up to April 2011. Since that time up till now the sputam, and respiration are yet continued. During the month of September 2013 a high heomoplysis happened after taking the Transamin Capsules of 500 m.g. it stopped and during the last winter season I took 2 spoons of honey mixed with milk and all the problems left, but during the hot season in May 2014 again heomoplysis started and on taking Transamin capsules stopped. It means that all the above mentioned are treatable and solveable as were solved on taking the honey. And again I am suffering from them. Un –fortunately I am living in such a country in which One M.N.A said on the floor of National Assembly that more than 50% medicines of our country are fake and we cannot do anything against them. I therefore request the Doctor like you of other country for help by suggesting the medicines of your country along with their email addresses/contact numbers etc. I shall be very thankful to you. Your’s sincerely Liaqat Ali Panhwar Officer National Bank of Pakistan Main Branch Dadu-Sindh -Pakistan

Pulmonologist 's  Response
Hello dear, thanks for your question on HCM.
I can understand your situation and problem.
Since you are having past history of tuberculosis and having recurrent haemoptysis (blood in sputum ), following possibilities must be rule out first.
1. Recurrence of tuberculosis.
2. Bronchiectasis
3. Scar bleeding due to excessive coughing.
4. Scar carcinoma rarely.
So better to consult pulmonologist and get done
1. Fresh chest x ray
2. Sputum culture for AFB and sensitivity to rule out MDR ( multi drug resistant) tuberculosis.
3. CT thorax to rule out bronchiectasis and carcinoma.
Treatment of each of the above are different. So we need to first diagnose the cause and then should start appropriate treatment.
At present , to stop haemoptysis, start codeine containing cough suppressant and tranexamic acid tablets.
Don't worry, you will be alright.
