Suggest Treatment For Recurring Cerebral Venus Thrombosis

I was diagnosed around 12 months ago after being very ill with cerebral venus thrombosis. I have been on warfarin treatment fro 6 months and at the end of November was gradually taken off the warfarin. Over the past three weeks I have been having dizzy turns and wake up most mornings with pain at the back of my right eye and nerve pain at the back of my head. The symptoms which I now have I had when I originally started to be ill over 12 months ago. Please can you give me some advice on what I should do. I am very concerned that my blood clot may have returned.
