Suggest Treatment For Red Ulcer Near Anal Cavity

I have one single red ulcer near my anal cavity. It is small, not painful, and never began as a white pimple or as a cluster of pimples. I went to the doctor where an np did an exam. She said she saw no bacteria under the microscope and drew blood for herpes, gonorrhea, and chlamydia tests. She said it looked like herpes to her but tests would have to confirm. But she also said I had anal fissure from anal intercourse and that the two sores looked different and she didn t seem sure. Like I said I don t have typical herpes outbreak symptoms and this would be the first outbreak if it were to be herpes. No blisters, it did not spread past the single ulcer even though I touched it. She mentioned syphilis at the visit but seemed to rule that out when she didnt see anything under the microscope. I do not have any vaginal symptoms at all. It hurts to have a bowl movement because of the anal fissure but that was present the day after the intercourse not multiple days to weeks. What do you think this is?

General & Family Physician 's  Response
Hi,

Red ulcer near anal cavity may result from sexually transmitted diseases like HIV, human papilloma virus, syphilis, herpes, chlamydia etc. Some underlying conditions like Crohn's disease, ulcerative colitis, other inflammatory bowel disease, constipation, anal sex etc., may lead to form ulcer in the anal area. Treatment depends on causes. You should undergo some investigations like blood test for HIV, HPV, syphilis etc., CBC, CRP, colonoscopy, sigmoidoscopy, biopsy from the ulcerated area and find out the underlying the causes which are responsible for your suffering and then treat them accordingly.

In the meantime, you can take Clindamycin for soft tissue inflammation, lactulose for preventing constipation; apply Hydrocortisone ointment on your affected area, weight reduction and abstinence from anal sex.

Take care. Hope I have answered your question. Let me know if I can assist you further.
