Suggest Treatment For Severe Abdominal Pain While Having Abdominal Adhesions

I have been diagnosed with abdominal adhesions from total colectomy (2007 plus 2 more surgeries that year to complete ileoanal anastomosis j-pouch) that have caused two episodes of partial small bowel obstruction for which I was hospitalized for 5-6 days each (2013 & 2016). Dr. says obstructions will likely re-occur & may require surgery at some point (which likely will cause more adhesions). After hospitalizations for PSBO, I have experienced sudden periods of severe abdominal pain with other symptoms (bloating, severe nausea, severe reflux, almost vomiting-mouth watering/drooling). I never know if I m going to end up hospitalized or if I will make it through the episode at home. The severe pain causes loss of sleep, which can lower my seizure threshold (seizure disorder since childhood). What is best for me to try to care for myself during these painful, disrupting, and scary episodes? Currently I take Bentyl with Ativan & then lie down for the abdominal pain. This often helps after a couple of hours unless it is a more severe episode which develops the other symptoms. Then it is much more severe & disruptive. I am fortunate if symptoms subside after 3-4 hours, with lower levels of pain remaining. What else can I do to manage these episodes at home (until I deteriorate, last time with delirium to require hospitalization)? I d like to stay home as long as I can and relieve these painful events, hoping they will pass. Past history with going to ER has been fruitless and extended painful episodes with no answers or suggestions except of course when a partial SBO was seen. Then a rough nurse forcefully crams an NG tube in my nose that adds more pain to my entire hospital experience. So, I know these episodes are like to recur and may even require surgery to cut out the adhesions. But I also have these episodes I ve described above, which occur without much notice & I never know how it will end up. I really want to be able to take care of myself during these episodes at home as much as possible. I realize that I will likely end up in the hospital for treatment & possibly surgery, but I need advice to enable me to get through these episodes better and to avoid hospitalization as long as possible. I have read your query and here is my advise.

* As per my surgical experience, post operative adhesions are very
vital cause of nasty constant ongoing dull to severe pain in various
regions of the abdomen depending upon the location and severity of
adhesions.

* My guidelines at present

- anti spasmodic medications as dicyclomine or drotaverine
- diet precautions to avoid menda, dairy stuffs
- plenty of liquids
- manage to have soft, smooth bowel evacuations with high fiber diet,
bulk forming laxative as Ispaghula husk or Psyllium
- control of body weight, blood sugar, cholesterol

- remain ambulatory
- regular exercises
- deep breathing and YOGA

* In certain cases, not responding to conventional conservative
approach, one may have to discuss about the indication of diagnostic
laparoscopy with adhesionolysis procedure of cure.
