Suggest Treatment For Severe Dizziness And Nausea

My 62yo girlfriend is having dizziness, the room is spinning while supine, and nausea. she has a distant history of positional vertigo but never with the nausea. I am a critical care nurse but not clear if this is something neurological or perhaps she is hypotensive. What do you think?

Hi welcome to HCM...

For dizziness blood pressure measurement and by capillary prick glucose estimation has to be done first ...(as you are nurse , so at home this two thing can be checked )
If hypotension or hypoglycemia present than treated accordingly ...

If dizziness occurs in particular position than it could be benign positional vertigo.
For which epley manouever useful..
If needed betahistine drug can be prescribed ...

Otological examination also needed ...
Labyrinthitis like cause can lead dizziness ..

Also investigate with HB estimation for rule out anemia ...

If neurological cranial nerve examination abnormal than more work up needed ..

Advise : otorhinolaryngologist consultation..
