Suggest Treatment For Severe Joint Pain

hi ,i had rheumatic fever developed when 14.ever since i had frequent joint pains untill i reached 21.everything was going fine but i am facing the same issue again.my chest is burning and and it is making feel really uneasy as if i have some heart issue.what shud i do now.
