General & Family Physician 's  Response
Hello dear Greetings
Regarding your experience with pain at your knee with difficulty in standing or walking with a weight of 290pounds I will suggest strongly that you have developed an osteoarthritis due to overweight

In view of this please do an x-ray of the knee and with the result you can schedule an appointment with an orthopaedic specialist for further clinical assessment and adequate drug prescription

Perhaps you will benefit from intra articular injection or a knee replacement surgery

I hope this helps Regards
