Suggest Treatment For Severe Lower Abdominal Cramps

I am 56 and no periods for 3 years I started hrt treatment evorel patch 7/8 months ago. 11 days ago at work I suddenly had a severe cramp type pain in my lower abdomen. It did pass off and the next day I has some blood spotting. It has carried on till today when it seems to have stopped. It was never much but was dark and then lighter in colour like a mini period. The only thing I can recall was forgetting to replace a patch last month so I went 4 days without one, could this cause me to bleed.
