Suggest Treatment For Severe Mouth Ulcers

I have twice now starting in the summer SEVERE mouth ulcers that also appear on my knees at pressure points as well as my elbows and tops of my toes. They last for MONTHS on end and the pain is unbearable! I no longer eat Wheat or Gluten and have taken dairy products out as well. I eat no garlic no Echinacea or vitamins except Bs and recently added 3000 ml of L Lysine to my diet to keep sores off the outside of my lips. I have been given Dexamethasone Elixir to help mouth sores but not working. If I pay the fee do you prescribe meds! YYYY@YYYY

General Surgeon 's  Response
Hello,

As per my clinical experience, the mouth ulcers are common clinical presentations in various GI upset conditions, particularly that of chronic ailments of malabsorption or others. Constipation with folate and B12 deficiency are the most common etiology for this presentation. My guidelines at present:
- Stop further doses of Dexamethasone, a major cause of concern in developing chronic mouth ulcers
- Drink more of liquids
- Consume high fiber diet with plenty of green leafy veg, pulses, legumes
- Avoid spices, non veg, junk food
- Use bulk forming laxatives such as Ispaghula or Bran
- Supplement of folic acid with vitamin B12 and zinc
- Pre and probiotics to support GI flora
- Regular aerobics and meditations
- Local application of mouth gel with Chlorhexidine content
- Gargles with salted lukewarm water added peppermint oil
- Avoid smoking or tobacco in any form if applicable

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Bhagyesh V. Patel, General Surgeon
