Suggest Treatment For Severe Pain Causing Sleeplessness

I have a conundrum. I have a herniated L4&L5 from farm and mechanics work since I was 8, torn meniscus in left knee, broken left toe from I was breaking horses when I was 15. I need the bone shaved down to fix it. I am 45 and wake up in the morning crying in pain and hardly work a 4 hour day let alone 8. I have a few beers and ibuprofen just so I can get some sleep and go to work. My doctor will not give me anything for the pain saying I drink too much. I need pain killers so I can sleep and go to work, I DON T want beer but it helps to sleep. I DO NOT do illegal drugs. What do I do ?

Use some gud analgesic dat is long lasting i e tab. Piroxicam 20mg once daily aftr meal or with milk fr 10 days. Use tab. Tizanidin 2mg at night fr 10 days. Use piroxicam gel on area of pain
