Suggest Treatment For Severe Pain In Buttocks And Rectum Area

Hello Sir, my name is kaleem, my age 30, I am suffering with savior pain in both buttocks and rectum aria and some pain in right leg this is my MRI Report
IV DISS
Loss of normal hyperintense signal intensity of nucleus pulpous noted in L5-S1
intervertebral disc on T2W images.
Annular disc bulge noted involving L4-5 intervertebral disc causing thecal indentation &
mild bilateral neural foramen narrowing.
Facet joints and rest of neural foramina appear normal.

IMPRESSION
     MRI finding are s/o
Degenerated L5-S1 intervertebral disc with posterior annular tear.
Annular disc bulge involving L4-5 intervertebral disc causing tecal indentation& mild bilateral neural foramen narrowing.

Please sagest me I need surgery ….
