Suggest Treatment For Severe Pain When Diagnosed With Endocarditis

I m in Shands Hospital right now being treated for my endocarditis. When I came in a few days ago, myself, to continue my antibiotics (get off the streets, save, & better my life) that I was supposed to complete six weeks of, but only completed three and a half weeks and had to move due to being homeless. I got the endocarditis clearly from being an user or from my abscess in my tooth causing severe migraines and I have been in two car accidents, I was just in a head-on collision on the 27th of September recently, they gave me one milligram of Dilaudid every 3 hours when I willingly came in here myself. After they did some blood test and an echo on my heart they pretty much laughed in my face (LITERALLY) when I told them that I have endocarditis & we re very nasty about the fact that i knew damn well i had/have it & said i 100% dont have it, but I was comfortable on what they gave me which was the Dilaudid. After they kicked me out of the hospital disrespectfully and unprofessionally laughing about my case, they called me back the next day proving that I was right. My blood cultures did in fact have bacteria the next day and I was positive for endocarditis. When I came back as they told me to rush in as soon as possible, the nurse was the first person I seen and talked to and she completely judge the fact that I am an addict and it only got worse from there she said whatever she assumed about me and judged about me to the doctor so they didn t give me anything as I was going through withdrawals and I m still going through withdraws today I m in severe pain I have crucial migraines from my teeth being so decayed in the back and having an abscess or two, have not got another MRI yet because I have dermal piercings in my chest so they refused to give me an MRI which they say they need to give me better pain medicine I ve been crying tossing and turning with drawing but the main thing is I need to have my antibiotics and stay here to save my life. How do I make these doctors or figure out a doctor that will not judge me & discriminate the fact that I was an addict because I would be out on the street doing whatever I want to do if I wanted to but I clearly want to change my life so I came in here to save it and better it even though I still believe very strongly that I need pain medicine of some sort for all the problems I do have. Is it my right to request a different doctor or the same one who made me comfortable the first time I was here? Because I have to be here for at least 4 weeks and I m completely miserable and depressed being in this pain and being judged and discriminated against and Shands Hospital in Gainesville. How can i genuinely get a doctor to understand my honest pain & migraines instead of assume/judge/discriminate & actually HELP my case. Even with my mother in the room the addiction doctors who have nothing to do with my endocarditis are pretty much threatening and harassing me about going to the methadone clinic and I don t have money for it and that s just another addiction I don t have transportation to get there all the time and that s the worst thing to withdraw from I ve experienced it before and I don t want to do it again. As soon as I did not agree could not get another word out of my mouth they stormed out of the room every single doctor is talking about going on the program with methadone instead of addressing my problem(s) at hand. I m not exaggerating when I say but they are literally harassing me to do methadone me and my mother do not want that I don t think that I need to go to a rehab this 6 weeks of antibiotics is pretty much how I know I m recovering butt going through withdraws is just making me want to leave and fix it myself when I m here for them to help me with that but not only that their reason for my using is due to depression, anxiety, and chronic pain that I ve had for a couple years and now it s only worse because of this new car accident please help me figure out how to save me pretty much and get the medicine I need prescribed!

General Surgeon 's  Response
Hello,

* Severe pain with endocarditis most likely is suggestive of pericardial friction rib via pericardial inflammation or else.

* A recent EKG and 2 D ECHO report of the heart would be best predictable with all details of the cause of pain.

* Let me know if I can assist further in this regard at any point of time in this case.

Hope I have answered your query. Let me know if I can assist you further.

Take care

Regards,
Dr Bhagyesh V. Patel, General Surgeon
