Hi,Dear,Thanks for your query to HCM.
Dear I read your query and reviewed it with context to your query facts.
I understood your health concerns and feel Concerned about them.

Based on the facts of your query, your girl friend seem to suffer from-
Testicular Pain with Erection-post traumatic Varicocele.
Seems that your trauma- 18 yrs back to right testicle, has precipitated the unnoticed Varicocele, which might be cause of recurring pain despite of medications from many doctors.
I would suggest to evaluate your testes with Consults from Urologist and USG studies.
Possibilities of Varicocele and / or Penile fracture with fibrosis / or could be with fibrosis, needs to be ruled out in your case before starting further treatment in your case.
Remedy-
Consults from Urologist.
Lcr(ligase Chain Reaction Test) testing-to rule out Chlamydia / Gonorrhea testing just to rule out other causes.
Any UTI from earlier post-traumatic effects to penile shaft and or / to epididymis needs to be ruled out.
Possibilities of post- traumatic spermatocele / epididymitis / needs to be ruled out and treated accordingly.
Remedy would be surgical correction, after fixing any of the cause as discussed above.this is because, medicines have not worked with many doctors, who treated you so far.

Hope this would help you to treat your health issues in the bes t way possible.
Welcome for any further query in this regard.
Good Day!!
