Hi.
Your history is self explanatory and needs a review by yourself.
Wince the cause of testicular pain is meth, you have to stop it as a treatment.
Get an opinion and clinical evaluation by Urologist;
ultrasonography and color doppler examinations to get the correct diagnosis and the treatment will depend upon the findings.
Diabetes:
This again is most important. Till it is under control, no medications will help you. You have to consult the Diabetologist and you look to be overweight or obese. This too needs a care and proper treatment.

I find this answer helpful
