Suggest Treatment For Severe Urinary Incontinence

Hi, I m a 51 year old female and having a severe urinary incontinence issue that s caused the surrounding external area of my vagina to be very painful. Lots of raw skin spots as well as white looking spots. When I cough even if I just used the bathroom I urinate a very large amount, so much I have to wear a pad that I frequently change. Also years ago I was diagnosed with letchins schlerosis (not sure of spelling) in which I have temoviate ointment. Would this ointment be useful in using to heal areas associated to the UI skin problems? What do you teccomend?

Hello and welcome to HCM,
The urine incontinence can occurs in number of local or bladder related disorders and conditions at spinal level.
The spinal nerves at the level of S1,S2 and S3 are responsible for control of bladder and any disorder at this level can lead to neurogenic bladder with symptoms of incontinence.
However, neurogenic bladder is seen in association with other conditions like Alzeihmers disease etc.
Urinary incontinence common in females is stress incontinence and usually occurs after multiple child births.
Incontinence after coughing is suggestive of stress incontinence.
Thus, it is first important to diagnose the type of incontinence and thus plan further management.
I suggest you to consult an urologist to diagnose the condition.
Treatment can be planned according to the type of incontinence.
The raw areas on genitals can be managed by using soothing ointment and anti-fungal in case fungal infection develops.
This will provide symptomatic relief but the ointment used for lichen sclerosis may not help.
However, the definitive treatment is treatment of urine incontinence.
