Suggest Treatment For Shoulder Dislocation

I had a motorcycle accident a couple of years ago and fractured my vertebrae and dislocated my shoulder. Now everytime i raise my arm above my head or even turn my head when shaving i start to black out! Would you know what is creating this problem? Thanks.
