Suggest Treatment For Shoulder Pain

Hi I hurt my lift shoulder about 12 years ago, it seems like near my shoulder blade, i believe it is the ligament as I feel that area so tough,it was softly painful but now It is getting more and more, while i work in the office I really feel discomfort, what do you think what should I do
