he most probable cause of soreness and enlargement of right testis with diabetes can be
Urinary tract infection going into the testis called
Orchitis.
I would advise you the following:
-Urine culture and sensitivity should be done before the antibiotic is started to get an appropriate results.
-Other tests of blood: CBC, urea, creatitnine, Blood sugar monitoring to keep diabetes under control.
-
Ultrasonography of the abdomen particularly for urinary tract, scrotum with color doppler mode.
-A long course of an antibiotic to be started and changed as per the report of culture and sensitivity.
-Scrotal support.
-Plenty of oral fluids.
-Not to hold urine to avoid retro-grade infection.
