Suggest Treatment For Spondylosis At L4-L5 And L5-S1

Dear Doc,

i have problem with low back pain since year 2000.last 2006 i have my MRI to update the status of my low back pain. in spite of the result issue to my disc. i did not seek for medical treatment.
But now I 'm severe suffering pain in my back through to my legs.my job was being affected due to this pain. Please see below result of my MRI since 2006.and give advice.

MRI LUMBO SACRAL SPINE
INDICATION: Chronic LBP
FINDINGS: Straightening of lumbar
Spine.
Reduced height and signal intensity of L4- and L5-S1 disc on T2...
at L4-5,there is annular disc bulge with left foraminal protrusion. There is also hypertrophied ligamentum flavum & face joint which is narrowing the spinal canal making it triangular in shape. the exit foraminas are narrowed as well specially the left foramina due to disc protrusion.
At L5-S1 also. there is annular disc bulge with hypertrophied facet joint & ligamentum flavum.in addition. there is right foraminal disc protrusion all of which is narrowing the spinal canal & exit foraminas specially the right one...
- Normal Marrow signal of examined vertebrae.
- Normal appearance of spinal cord, caudal equina and filum terminale..
- Para signal soft tissue is normal.
IMPRESSION: Spondylotic changes at L4-5 & L5-S1.....
Please advice ?????
