I sprained my ankle really badly around five years ago. I am getting increasing pain in it. It is not constant but if I step on it in a certain way I have a sharp stabbing pain and on long walks it is increasing painful. Is it likely to be scar tissue, should I see a physio?

A sprain 5 years back, still causing pain indicates severe sprain injuring some ligament, fracture of all bones must have cause malunion of fracture or pinching damage to the nerve.
You need an orthopedic surgeon, imaging investigation like MRI to pinpoint the cause followed by specific needful management.
