Suggest Treatment For Swollen And Painful Tonsil

Hello Good evening Doc, I am living and working here in Dubai, UAE. I consult a doctor at NMC hospital last September 23 2014. the name of this Doctor is Dr. Mohamed Akram Metwally (M.B.B.CH,MD (Otolaryngologist). his findings is I had Chronic tonsillitis. then he advice me to take this medicine. CLARITHHROMYCIN. and gargle warm-salty water for my throat / (tonsillitis ). And he advice me to come back September 28, 2014 to remove both of my tonsil by using laser. but when I take medicine in 2days the swelling gone. and then after 6days I experienced severe pain and swelling of tonsil. I can t talk properly because is cause pain when I speak long time. then when I check it using mirror I saw small and tiny things inside / both side of my tonsil and it get more reddish look and swelled... and I go back yesterday October 4 2014 Saturday at 6pm. after EID here in Dubai, I look for Dr. Akram but he wasn t there at NMC hospital. so they refer me to other ENT doctor His name is Dr. Yeshwanth Chakravarthy Specialist Otolaryngologist U/S his findings was different he tells me he didn t see any problem about my problem tonsil and he says no need to remove your tonsil . but he put on his diagnosis is ACUTE LARYNGOPHARYNGITIS . Then he gave me prescription CEFTRIAXONE 1 G POWDER FOR INJECTION FOR IV(1+1, VIAL+ SOLVENT AMPOULE DEXAMETHASONE 4 MG/ML SOLUTION FOR INJECTION (50 X 2ML, AMPOULE) CEFUROXIME [500MG] 500MG TABLETS. PREDNISOLONE 5MG TABLETS (20 S, BLISTER PACK). Now my question is. Doc, is this proper medication for my concern sickness? for having tonsillitis? Many thanks. Looking for your Immediate response. I understand your concern and I would like to say that tonsillitis and laryngopharyngitis both are infective diseases of throat and treatment regim is also similar in both situations. Here I would like to say some times Laryngopharyngitis is a complication of tonsillitis.

For clarification of your confussion about diseased tonsil good clinical examination is necessary. I would like to tell you signs of chronic tonsillitis.

1- Tonsil should be enlarged.

2- Anterior pillar of tonsil should be congested.

3- Either septic focus will be on tonsil or on pressing tonsil infected discharge will come out.

4- Some times jugulodiagestric group of lymph nodes will be palpable.

All above signs may be clearly seen by your doctor.

from the history and examination i think you are suffering from chronic tonsilitis
if you are able to take orally then there is no need to take injectable antibiotics and steroid
you may take oral t.amoxyclave 625mg 1bd for 7 days along with analgesic and antacid.
and if you are suffering from recurrent attack of sore throat you should undergo tonsillectomy
