Hi,

Noted the history about your husband about having gout with pains in toes, ankles, knees which lasts for 14 days with a god days of 2 or so, now has developed difficulty in swallowing food but not informed the Doctors, he is 68 and overweight.
With the history you have provided, I can think abut the following as the possible causes.
Difficulty in swallowing with the feeling of something sticking in chest indicates obstruction in the food pipe that is esophagus.
The causes further can be benign like stricture or can be cancer or so.
Hence He should consult a Doctor and get a reference for Barium swallow, Upper GI Endoscopy and biopsy.
Once the diagnosis is clear the treatment will be specific.
As for pains in many joints, consult a Rheumatologist for opinion, proper and full investigations to get the correct diagnosis so that an appropriate management can be done and modulated as per the diagnosis for difficulty in swallowing.

Take care. Hope I have answered your question. Let me know if I can assist you further.
