Suggest Treatment For Uncontrolled Diabetes, Chest Pain And Breathlessness With High BP

I have had uncontrolled diabetes type 2 since my father passed away last year. I have been havig chest pains on left side (hurts on top rignt of medisertnum and another place is below left breast). I have been awaken during the night trying to catch breath. I am on bishor hctz for my blood pressure. It usually runs 138/97 or above.I work a lot and travel so I don t know if fatigue is from that or my chest problems. When looking up heart disease which runs in my family (Strokes and Heart attacks) I have all five symptoms. Other than an aspirin a day and trying to get 7-8 hours of sleep (which is hard traveling) along with my metformin, januvia, and bishor, what do I need to do and watch for?

General Surgeon 's  Response
Hello,

The uncontrolled diabetes (HBA1c above the level of 7.5 mg/dL) is the root cause of the symptoms of chest pain and breathlessness with high blood pressure. Alterations in sodium intake, stress levels with renin angiotensin aldosterone system secondary changes do bring extra problems over and above the diabetes part.

My recommendations for better recovery:
- Strict control of diabetes with a goal of HbA1c below 6
- Dietary care, regular exercises
- Meditations and YOGA
- Control of serum cholesterol (target LDL below 70)
- Prophylactic anti-platelet or blood thinner
- Control of body weight
- Strict avoidance of smoking and alcohol if consuming
- Regular visit to your physician

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Bhagyesh V. Patel, General Surgeon
