Suggest Treatment For Vaginal Thrush And Fissures

I have been suffer from reoccurring thrush for the past 2 years. Over the last 3 months it has got particularly bad. I have taken many over the counter creams and tablets, but it only subdues the symptoms for a week or so then it comes back. I am also suffering from vaginal fissures, is this due to the thrush and what should I do to improve the situation. I recently visited my GP and they gave me a weeks course of antifungal cream which is the same as i have purchased over the counter. I am at my wits end.
