Suggest Treatment For Varicocele On The Left Testicle

Hello, I am a healthy 26 year old male. I was diagnosed with a varicocele in my left testicle over 10 years ago, and apart from the occasional dull ache, I ve never had any problems with it. For the last 2 weeks however, I ve had an intermittent ache in the veins that sometimes spreads to the kidneys, not particularly sharp but more painful than normal, and for the last two days the pain has been noticeably more intense, beginning very mild at the start of the day and becoming very uncomfortable towards the evening, after work. Since this morning I have also been experiencing lightheadedness when I stand up, and feel a constant mild discomfort in my left testicle no matter what posture I take. I have made a doctor s appointment for tomorrow, however the lightheadedness has got me slightly worried. Can you please advise?

General & Family Physician 's  Response
Hi,
Thanks for posting in HCM.
I have read your description and understand your concern.
Varicocele is an abnormal enlargement of the pampiniform venous plexus, which drains the testicles, in the scrotum.
It is quite common affecting about 15 percent of male population.
The condition can have an effect on fertility. It affects about 35 percent of men with primary infertility and in 75 to 80 percent of males with secondary infertility.
It’s not always necessary to treat a varicocele.
Treatment is usually instituted if it causes:
1. pain
2. testicular atrophy
3. infertility or thinking about assisted reproductive techniques
Treatment options available include:
1. Varicocele surgery, which is performed on out-patient basis and may improve fertility.
2. Embolization, which is alternative to surgery, which is a minimally invasive treatment, generally performed by an interventional radiologist.
Hope the information provided would be helpful.
All the best.
