Vitiligo is a skin condition characterized by loss of melanin thus leading to the formation of white patches.
Treatment is guided by the severity/ extent of involvement and also by the activity of disease i.e whether it is stable or spreading.
Your wife seems to have widespread involvement with vitiligo and it seems to have spread i.e it is active.
In actively spreading generalized vitiligo Oral
steroids are the drugs of choice. They are effective in halting the spread of patches besides re-pigmenting them.
In addition to systemic steroiods other treatment options that is beneficial for widespread vitiligo is
phototherapy in which the patient is subjected to narrow band UV B (311-313 nm wavelength) in a closed chamber, twice or thrice every week.
Multiple sessions may be required and treatment may extend to months.
In addition to systemic steroids and phototherapy topical treatment with moderately potent topical steroids like
mometasone furoate and Topical calcineurin inhibitors like tacrolimus is also part of therapy of vitiligo.
