Suggest Treatment For White Substance On Tongue

Hi I would appreciate your help. I have a white substance on either side of my tongue. It does not appear to be thrush. I have been tested for aids and they have been there for a while. Just today purple areas appeared next to them, especially on the right side. The upper part of my tongue is often very irritated or sore.
Is this type of thing always cancer or autoimmune illness?

Thanks for your query, I have gone through your query.
The white portion can be a frictional keratosis or lichen planus. Nothing to be panic consult a oral physician and get it evaluated. If it is a frictional keratosis, we can remove the causes like smoothing the sharp cusps of teeth. If it is a lichen planus we can use topical steroids like triamcinolone acetonide. The purlpe area can be a enlarged or dilated vein.
