The glycaemic control in
diabetes mellitus is obtained gradually through a combination of diet,exercise and medications.Being non compliant with the medicines may lead to chronic complications so it is very necessary that you you take your medicines as prescribed.There is no use if you fail to take it because of some reasons and then go on adding the drugs without physician's consultation aiming to achieve a fast lowering of blood sugar. Doing this is dangerous as it may cause
hypoglycaemia along with other drug interactions.Therefore,if I were your treating doctor,I recommend,
-treating with first line treatment with drugs glipizide and metformin twice (prescription drug)
-Advice regular exercising like brisk walking for 30-45 minutes atleast 4 times in a week.
-Adequate balanced diet.Avoiding sweet,salty,processed,refined,high glycaemic index food items.
-Avoid smoking and alcohol
-3 monthly follow up for blood sugar ,
blood lipids,body weight and ruling out other complications and 6 monthly follow up for glycosylated
haemoglobin.Depending upon blood glucose control,I would add further drugs like
sitagliptin.
-While on oral hypoglycaemia drug,keep a watch over signs of hypoglycaemia like sweating,tremors,palpitations,giddiness and keep some eatable with you containing sugar in it.
