Testosterone Low. Take Hormone Therapy, Pituitary Blood Results Normal. Told The Brain Is Not Picking Up Signals And Testosterone Is Not Generating. Advice?

My testosterone level is 261 I'm 38 years old. Have had it low for about 3 years now, went to my endocrinologist last year and he put me on a hormone therapy, using hcg injections alonside clomid. Helped me out a lot was on it for 6 months and levels went up to 500-600 range before stopping in sept.of last year. Went to primary care and now at 216..all my other bloodwork panel is comes back perfect..what can be causing this? Pituitary bloodwork is normal as well, he said my testes or brain must not be picking up signals at receptor and I'm not producing testosterone on my own.

General & Family Physician 's  Response
Hello,
You had low testosterone levels. It can be Primary (testis) Secondary (Pituitary) Tertiary (hypothalamus). The causative agents to be assessed. Generally low testosterone levels in the body can be supplemented by hormone replacement with testosterone. Testosterone replacement therapy can be prescribed as an intramuscular injection usually given on a biweekly basis, as a patch or gel placed on the skin, or as putty that is applied to the gums of the mouth.
I hope you understand the treatment.
Best wishes

General & Family Physician Dr. Luchuo Engelbert Bain's  Response
Hi and thanks for the query,

I strongly advice you seek the services of a urologist - andrologist and an endocrinologist. Coordinated aspects of both nutrition, medicine and psychotherapy are required. I am afraid its however rare for responses not to be observed. Psychological issues could intervene. Some patients do develop side effects. A good liver function state is required before this treatment is instituted. You might need a good clinical review from these specialists. Thanks and kind regards.
