What Are Side Effects Of Fetzima?

My daughter is taking lithium. 300 mg once a day and amitriptyline Hcl 25 mg at bed time and topamax 100 mg twice a day. On Friday her doc put her on Fetzima 20 mg for 2 days then increasing to 40 mg the next 5 days. She has been very sleepy and over all does not feel well. She has lupus and fibromyalgia. She is 30 yrs old. Should she take the 40 mg tonight or wait until tomorrow and call the doc. My concern also she is taking Fetzima and Lithium. Is this ok?

General & Family Physician 's  Response
Hi,

Fetzima (levomilnacipran) is a selective serotonin and norepinephrine reuptake inhibitor antidepressant (SNRI) commonly prescribed to treat major depression and bipolar disorder. Its common side effects include nausea, and orthostatic hypotension. Other side effects include: vomiting, constipation, increased heart rate, palpitations, vomiting, delayed.

When given along with lithium and or amitriptyline it can increase the risk of a serious condition called the serotonin syndrome, which is characterized by confusion, hallucination, seizure, increased heart rate and blood pressure, fever, excessive sweating, shivering, blurred vision, muscle stiffness, tremor, stomach cramp, nausea, vomiting, and diarrhea. In view of the above I advise you to stop giving Fetzima tonight and consult her treating doctor for an alternate effective but a safer medicine to treat her condition.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Mohammed Taher Ali, General & Family Physician
