Hi,
When a trans-vaginal sonogram does not detect a gestation sac one week after a missed period while on regular cycles, it means you are not pregnant. This delay in the period could be due to hormone effect as evidenced by the bulky ovaries and thickened endometrium. You can also cross-check with an estimation of the serum beta-hCG titres and a repeat sonogram. If still negative, you can give a kickback to the ovaries with a course of hormones to induce a period. Hope this helps.
