my sister went to an std clinic and had a pinprick test for hiv, the nurse told her that her test was active but they were sending a bloodtest off to investigate further and then they sent her into a room with councilors does this mean she is hiv positive?

You had not mentioned the age of your sister, her marital status and sexual activities. All the test for HIV in the market are highly sensitive and reliable up to 98%. Depending on her activities and her partner sexual activities, we can express the results more confidently. When there is no risk for HIV/STD, then a more confirmatory test like western Blot or DNA PCR may be necessary to confirm or rule out the reactive test.
Counselor would prepare her to face the result. If negative whether it has to be repeated and about safe sex in future etc. and if reactive, he will guide her how to accept the same and what next has she to plan.
