Hi & Welcome to HCM.

Your sister seems to be taking optimal treatment for Rheumatoid arthritis.

1. SAAZ (Sulfasalazine) is a disease modifying anti-rheumatic drug (DMARD) which not only reduces the pain and inflammation associated with rheumatoid arthritis but also retards the ongoing disease process.
Generally it is a safe medication, however it may cause nausea, vomiting, headache, leukopenia (decreased white blood cells count), megaloblastic anemia, and allergic reaction (rash or itching) due to its sulfur content.

2. Folitrax (Methotrexate) is one of the most effective and safe drugs used in the treatment of Rheumatoid arthritis. Its most common side effects include: ulcers in oral cavity, inflammation of the liver & kidneys, increased risk of infection & anemia.
These side effects are related to inhibition of Folic acid by Folitrax, hence they can be prevented by adding Folinic acid tablet.
She also needs to take green leafy vegetables as they contain good amount of folic acid so as to overcome these side effects.

She needs to have periodic lab tests such as CBC, kidney and liver function tests to monitor the side effects of the above listed medications.

3. HCQS (Hydrooxychloroquine) is another important drug used in the treatment of Rheumatoid arthritis, its use is associated with headache, irritability, visual impairment due to retinopathy or corneal deposits.
She needs to have periodic fundoscopic examination of the eyes to detect these eye related side effects.

Since these drugs are slow acting as it may take 4-6 weeks to have their maximum therapeutic effects, she may take non-steroidal anti-inflammatory drugs or steroids along with these medications after consulting her treating physician or rheumatologist if the pain and swelling is causing her discomfort.
Hope the information given above could be of benefit to your sister.
