You are sufferening from a condition known as "urticaria" which can be acute or chronic.
If it is more than 6 weeks, it is known as chronic urticaria.
It is a sort of allergy which can be due to infection or any drugs.
Please tell me if you are on any sort of medication other than the ones you told me.
This sort of allergy can be severe enough leading to 
respiratory distress and 
angioedema that is swelling of lips and eyes; which can be an emergency situation, may require urgent hospitalisation and intervention.
I will advice you to get your 
hemoglobin, Total leucocyte count , differential leucocyte, urine routine and microscopy done.
I will recommend you to take tablet levocetrizine in night and tablet 
fexofenadine in morning.
You may need to take a short course of oral 
steroids but under supervision only.
