What Can Heavy Bleeding Imply?

I am on my period, got up this morning to use the bathroom and thought I was bleeding on myself, going down my leg. When I made it to the restroom, I realized it was clear my panties soaked, my tampon had blood that was old but soaked from what ever this was. I know for sure I did not pee on myself. What is this?
