What Causes A Delayed Period With Tender Breasts?

I just turned 40, my periods have always been 28 days exactly, never late, unless pregnant. I am currently 1 week late, have had 4 negative urine tests. Am having intense stabbing pain periodically in the vagina and tender breasts. What on earth???
