What Causes A Grinding Sound In The Shoulder After A Rotator Cuff Surgery?

my husband has had rotator cuff surgery on May 31st. One of the muscles tore badly and could not be reattached. He has been in chronic pain and now has a gritty or grinding sound in the shoulder when he moves it up and down. What could be causing this gritty sound? Thank you.
