What Causes A Large Fluid Filled Painful Lump On The Stomach?

I have about a glof ball size lump on my stomach and soft and almost fluid like in the center and hard all around and all of it hurts i had it about 3 days now im 27 im smoke never had one of these before what should i do ive taken otc pain meds and tried warm compress should i just go to the er
