General Surgeon 's  Response
Hi,

Noted the history related to 74 year old female with scoliosis, sciatica, headache and a movable tender mass in abdominal wall.
All the history is suggestivee of a probable hernia that is obstructed hence causing the pain and tenderness.
I would advise you tthe following in such a situation:
Consult a General Surgeon for the abdominal hernia as this may be an emergency so that a proper clinical evaluation,m examination and supportive investigatoins like CT scan of abdoen can be done.
At the same time needs review by a Physician and Orthoedic Surgeon about scoliosis, sciatica and MRI for the same.
She may need an urgent surgery for hernia to avoid further complicatoins of strangulation and so on.
Once the hernia part is over then concentrate on the sciatica part so that an appropriate spine surgery can solve her sciatcia and scoliosis problems at least to some extent reducing the needs for medicatoins.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. T Chandrakant,
General Surgeon
