What Causes A Painful Lump In The Jugular Notch Area?

I have had a lump in my jugular notch area since October 2014. I was discharged from hospital over six months ago as it gad not changed in size. I was asked to come back if my the lump changed in any way or my voice became raspy (which it does on occasion. In the last four weeks it appears to have grown and feels as though it is spreading towards the right side of my beck. It is causing me different sharp pains in my neck n head as well as a pressure headache. It hurts when I cough, sneeze, swallow, move n sleep ( which keeps me awake).
