What Causes A Tissue Like Discharge From Vagina?

Hi I'm 33 years old and for the past 8 months or so I would have tissue coming out of my vajayjay whether it's after sex or not. I have not been sexually active for the past month or so but I when wipe I would still see tissue and sometimes it has a sour smell, a couple of times it was and odd color and during my period sometimes it's the size of quarter to half dollar. There's no itching but I know something is not right. What could it be?
