What Causes Aches At The Back Of The Head?

I am a healthy woman of 61 and have head aches around the back of my head. No ear infections since age 6 and no ear audio loss, pain or drainage ever. I had an MRI and was informed I have mild bilateral mastoiditis.
Will I need surgery to correct this situation or will local antibiotic injections be used to correct the infection in the bone?
