What Causes Anal Itching, Abdominal Swelling And Pain In LLQ And RUQ?

PT age 26 (not 29) Black, male c/o anal itching, abdominal swelling, pain in LLQ and RUQ, intense periodic abdominal and rectal pain, swollen lymph nodes in the pelvic area. Frequent changes in stool appearance from narrow to fibrous to tarry, but not necessarily bowel habits. CT Scan presents no abnormalities, there was no indication of a need for MRI and general blood work presents as normal, especially CBC. Formerly a pack a day smoker, now less than a pack a week with full intent and indication of complete cessation. Current medications include Risperdal 2 mg, Lamictal 100mg (1qd) Buspar 1 po BID for Bipolar Depression.

Hallow my friend,

Your gastrointestinal symptoms typically suggest some infestation - could be amoebiasis, worms or both. LLQ pain suggests the colon swelling (mostly due to amoebic colitis) while RUQ suggests swelling of the liver (mostly due to amoebic hepatitis). Itching around the anus could be due to the worms.

Better get your stools examined from the pathology laboratory. Please have an overnight purge, discard the morning first stools sample and collect the last portion of second sample for examination. Report to the physician with the reports.

Amoebiasis can be treated with metronidazole derivatives while worms may be treated by albendazole group of medicines.

Your statement of pelvic lymph nodes is confusing. If scan shows no abnormality, how could you see these nodes? Or do you mean inguinal (loin) lymph nodes? For that some more history is needed, particularly of sexual exposure as they are commonly involved in the infections of genitals or legs. You may require some antibiotics for that. If you ask me a direct question with the details, we may discuss the issue. Meanwhile report to the physician about the lymph nodes.
