General Surgeon 's  Response
Hi,

Noted your history of losing balance while walking, playing golf particularly on tilting the head down, turning fast to right and left, also feeling lightheaded.
This condition is called Vertigo.
As per the history you have provided the cause can be in the cervical spine causing kinking of Vertebral artery and causing insufficiency of blood supply to brain or can be due to problem with semicircular canals in inner ear.
This can be diagnosed by the following ways.
Consult an ENT Surgeon for clinical examination, ENT Endoscopy and relevant investigations like MRI of the inner ear and Cervical spine if cervical spondilitis is thought about.
Second opinion of a Neurologist will help further to get a proper diagnosis and have appropriate management.
Till then stop playing golf, do not walk alone, avoid all activities that cause Vertigo.
Treatment will be prescription based.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. T. Chandrakant, General Surgeon
