What Causes Black Lumps And Skin Tags On Left Testicles?

I just noticed about a dozen or so black lumps/lesions on my left testicle. I have several skin tags and it has been hurting the past few days so i assumed it was just a skin tag that had become irritated. When I was looking at it just now I noticed it was a black bump in the middle of a larger white bump surrounded by many more of these some all black some white bumps with black bumps on them? Should I be concerned and what kind of doctor should I see, a dermatologist or a urologist?
