What Causes Bleeding Ever After The Periods?

i had my monthly flow two weeks ago but after that i started having almost after two days blood stains on my panty and it has been on for two weeks now ,i consulted a doctor and he reccomended papsmear ,the test could not be carried out due to blood clots what could be the cause of this be ?
