What Causes Bloating, Lower Back Pain, urgency To Urinate And Shortness Of Breath?

Ok. So a week ago (June 8, 2016), I drunk a 20 oz orange juice without having any water or food in my body. An hour after drinking the oj, I could feel the uti symptoms (urgency to urinate and some bloating). I immediately drunk 70 oz of water for the next 3 hours. I drunk 70 oz over the next two days. I didn t have anymore bloating but still had the urgency to urinate. So on June 10, 2016, I decided to go to immediate care to make sure I didn t have a UTI since I had urgency to urinate. Urine sample came back good. No proteins, normal amount of red blood cells, no pus cells, normal sugar levels and etc. Doctor gave explanation for urgency to urinate was the amount of water that I drunk so no antibiotics needed. He recommended to keep drinking water and cranberry. On June 13, 2016, I still had the urgency to urinate (still no burning, no pain, no blood, no other issue). I then decided to have the doctor call me script and I was given Pyridium. I had to stop taking the script because the side effects was shortness of breath, dizziness, and confusion. Needless to say, no changes with urgency. June 15, 2016 I made a Dr Appt because still had urgency to urinate, some lower back pain briefly and alot of bloating (still no burning and no other issues). I had another urine sample. Results: A trace of bacteria and 8-10 amount of blood. Normal amount of blood is 0-5. Didn t see any puss cells. The Dr is treating me for a UTI but he thinks my bladder is inflamed and irritated more than a UTI. He prescribed Macrobid for antibiotic. He felt on my stomach and he didn t find anything alarming. If the antibiotic doesnt work, He thinks something maybe enlarged pushing down on my bladder causing the urgency to urinate. He was thinking my ovaries could be enlarged but I had an extensive pap smear in March 2016, results normal and ovaries feel normal. My mentrual cycles are normal and on time. No history of any female issues. I have to come back in 10 days for another urine sample to see if blood amount is normal. As of now, I am having severe bloating( stomach a little tender but no pain just severe discomfort). When my stomach swells, I have shortness of breath and some lower back pain. I take AZO for the bloating, works but comes back hours later. Not sure if its anxiety for shortness of breath but I am so concerned about the bloating and shortness of breath. Also, the urgency to urinate is not as strong so it feels like its going away. I have had a UTI in the past with bloating but never like this. What can be causing the severe bloating, lower back pain, urgency to urinate, and shortness of breath? Are these symptoms consistent with an irriated bladder?

General Surgeon 's  Response
Hello,

As per my clinical experience, the persistent bloating with urinary discomfort (in absence of urinary infection documentation) indicates most possible interstitial cystitis case. In my clinic, after thorough clinical examination, I do recommend a diagnostic cystoscopy for such cases. Till that time, continue hydration and addition of proton pump inhibitor as omeprazole or rabeprazole to be taken on empty stomach with sips of water will help nausea symptoms.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Bhagyesh V. Patel, General Surgeon
