What Causes Bloating,pain And Irregular Spotting?

I am a 30 year old female with history of constipation, recently worsening with extreme bloating and gas, and pain on intercourse and some unusual spotting as I have mirena IUD and do not have periods. I do have PCOS, and received fertility treatments in which I had triplet daughters in 07. Flat and upright of abdomen negative, and upper abdominal u/s negative as well.
